MUST I APPLY IN PERSON?
   
  You must always appear in person if you are 13 years of age or older and applying for your first passport. A parent or legal guardian must personally appear and execute the application for a child less than 13 years of age. A minor, 13 years of age or older must appear in person to execute the passport application. A parental consent form is required for minors. All individuals, including newborns, are required to have a passport issued in their own name. After January 1, 2001, a new law will be implemented which will require the signature of both parents on all applications for minors under the age of 14.
