MARRIAGE LICENSE APPLICATIONS: |

Both parties
must appear at Probate Court in person. Powers of Attorney are not accepted. Applications will be accepted from 8:00 a.m. to 3:30 p.m. No applications will be accepted after 3:30 p.m. |

Please bring with you
the following: |
| |
1.) |
Proof of identification - either a valid driver's license, a
state issued photo I.D., or a valid passport. If your address is NOT the same as it appears on your ID, you must provide proof of residence. |

2.) |
$ 55.00 cash - no check or credit cards. |
| |
3.) |
If you have been married before and are divorced or have had your marriage dissolved, you will need a CERTIFIED copy of your divorce entry or dissolution
decree with the raised Court Seal. Photocopies are not accepted. |

4.) |
If your former spouse is deceased,you must provide a certified copy of their death certificate. |

5.) |
If you have had a legal name change, you must provide a certified copy of the court entry changing your name. |

| |
If you have all the necessary
documents, your license will be issued while you are at the Court
and is valid for sixty (60) days. |

|
| |
Your license must be
obtained in the county in which you reside, and is valid anywhere
in the State of Ohio. |

|
| |
Out of state residents
may obtain a license in Columbiana County, IF they are to be
married in Columbiana County. The license is valid ONLY in
Columbiana County. |

| |
If applicants are under
the age of 18 they must have consent of BOTH biological parents
and a letter from a minister or certified counselor showing that the
applicant has received marriage counseling. If one parent is deceased,
you will need to provide a certified copy of the death certificate.
If one parent has sole custody of the minor applicant, you must provide
a certified copy of the custody papers. |

18, an appearance before the judge is required. |

CERTIFIED COPIES OF
MARRIAGE RECORDS: |

Approximately 7 to 10 days from the date of your marriage, you can call the Probate Court to inquire if the person who has performed your marriage has mailed the return to the Court. Once this has been received, you can order certified copies if your marriage record at a cost of $2.00 each. |
